Rates, Office Policies, and Consent Forms:

As a Senior Training Clinician, I set very high ethical and professional standards and will provide you and your loved ones with excellent care

Fees:

 

Individuals: 

My professional fee for the Initial Evaluation session is $350.

Ongoing virtual and in-person psychotherapy sessions are $300.

Couples Therapy:

                                                                                                                 Couples therapy is $375/hr.                                                                                                                     

If you would like more or less time, I will prorate my fee for you. 

I do offer sliding scale rates when available;  please inquire before our first meeting. 

 

 Payments:

Payments (cash, check or Zelle) for my professional fees are due before or at the time of service.

 

 Insurance:

I am not on any insurance panels and do not work with insurance providers.

However, if you have a PPO insurance plan (one that covers "out of network" providers), my services should be covered.

Per request, I provide monthly invoices for you to submit for reimbursement.

       

 24-Hour Notice Cancellation Policy:

When we schedule an appointment, you agree to pay the full fee unless you

provide me with at least a full 24 hours notice to cancel or reschedule.

 Please open and read the following forms to make sure you understand and are in agreement with all my office procedures.

 

Please print, complete and sign my Practice Policies and Consent for Treatment Form

 and bring it in with your payment when you come for your first session so we can get started with the interview.

 

Anxiety, Depression, Encopresis, Nightmares, Sleep Problems

Obsessive Compulsive Behavior Issues around Self-Esteem, Gender or Sexual Identity

Destructive, Oppositional, Aggressive Behavior ADD/ADHD, Learning Disabilities

Eating Disorders, Weight Problems Peer and Social Problems, Isolation or Alienation

Suicidal Feelings or Gestures Divorce, Grief, Loss or Trauma

Self-Destructive Behavior, Cutting Adopted, Step Children and Attachment Issues

Drug and Alcohol Use, Precocious Sexuality Health Issues, Chronic Illness
